By submitting this survey, you understand that it is your responsibility to locate suitable and eligible housing before the expiration date of your voucher. You understand the rules of the program and will comply with them as long as you participate in the program.

Once you submit this survey, this initiates the next steps of the program and will count as your official date for your voucher. You will have 120 days to utilize your voucher and may request one additional extension of 60 days, in writing, prior to the expiration date of your voucher. Extensions beyond 180-days must be approved through Reasonable Accommodation or Extenuating Circumstances.
