SECTION 4: Finding Caregivers and Community Connectors

The Caregivers in the Hills program is a FREE program established by Hills of Headwaters Collaborative Ontario Health Team and its Community Wellness Council to support unpaid caregivers in Dufferin/Caledon as a critical part of the health and social care team of their loved one or someone they care about. Promoting a positive partnership between unpaid caregivers and the health care community will ultimately improve outcomes for both caregivers and their loved ones. Learn more here.

Our Community Connectors group is just getting started. We are building a network of people in the community who would be candidates for a group of community connectors or "Super Connectors" in Dufferin-Caledon. These are people who are connected to many things themselves and have a knack for connecting other people in their neighbourhoods. The goal is to create more connected communities and a stronger social fabric. If that sounds like you or someone you know, please join our newsletter and we will reach out with next steps.
