Waiver: The undersigned acknowledges that outdoor recreational activities and fitness classes include some degree of risk. Thus, by participating in Town of Seven Devils Parks and Recreation activities, by signing below I (on behalf of myself and any minor under my care) acknowledge and accept such risks and hold harmless the Town of Seven Devils, Volunteers, and Town Staff for any and all injuries or damages resulting from participation in such activities.

Photo Permission: The undersigned (on behalf of myself and any minor under my care) grant permission to the Town of Seven Devils Parks and Recreation to use my likeness in any photo taken during Town recreation activities and fitness classes for the purpose of publicity of the Town of Seven Devils activities.

Emergency Contact: The undersigned (on behalf of myself and any minor under my care) provide an emergency contact below in the event of the need for such contact.
