Youth 360 Activity Scholarships – Application Form for 2023-24 School Year

Youth 360's activity scholarship program is designed to help Mat-Su teens stay involved in structured activities outside of school hours. We believe that youth who participate in positive activities are more likely to perform well at school, build connections with peers and supportive adults, and avoid negative influences. As of the 2023-24 school year, any student across the Mat-Su can apply for scholarships.


For school sports and activities, Youth 360 will cover up to $125 per activity for approved students. For community-based activities, Youth 360's payment will vary depending on activity and level of participation. After receiving this application, Youth 360 will contact parents to determine eligibility.


By signing up for this program, families agree to:

  • Contact the school or activity provider to enroll your student in the activity.
  • Complete all required registration forms, waivers, paperwork, for the activity.
  • Ensure your student participates in the activity. Dropping out of an activity, or frequently missing practices/sessions will make your student ineligible for future scholarships.
  • Complete a short survey to help us determine activity quality.

Parent/guardian responsibilities:

I understand that I am responsible for completing any additional paperwork or requirements from activity providers, and for working to make sure my child attends activities as scheduled. I understand that repeated no-shows may make my child ineligible for future Youth 360 stipend programs. I understand that my child’s participation in these activities is taken at my own risk and that Youth 360 is not responsible for the well-being of my child while participating in vendor or school-provided activities. I agree to complete a short post-activity survey to help Youth 360 determine activity quality.
