  • Your responses to this questionnaire will offer us an overview of the types of resources you may be willing to provide to ZDR3 Network facility members who have been impacted by an adverse event. Filling out this questionnaire does not obligate you to deploy resources.
  • If you own facilities in multiple locations, if your different locations plan to manage and deploy resources and responders separately, each one will need to have its own responses to this questionnaire.
  • If you have expressed interest in providing support, upon completion of this questionnaire, we will be in touch for follow-up information about the types of resources you may be able to offer.
  • We understand that resource availability is ever-changing, but this questionnaire will help us better understand your facility’s potential capacity.
  • Any information submitted will only be accessible by the core ZDR3 communication and command staff. It will not be made public or disseminated.
