Participant Feedback Survey

Thank you for your willingness to complete the Participant Feedback Survey! If you would like to be entered into our quarterly raffle, please be sure to answer all questions before you submit and make sure your email matches the email address on your Record Book application.


One winner will be drawn each quarter ahead of the release of the CAF Quarterly Newsletter, in the months of January, April, July and November to receive a $100 Visa Gift Card. Only registered participants can receive prizes from the monthly raffle.
