OCES will spotlight Caregivers in honor of National Caregivers Month. We invite you to nominate a Caregiver (individuals, family members, friends, associates, others) who have positively impacted your life or someone else’s life within OCES’ service area. Please read and reference the nomination criteria section below prior to completing the form. Thank you in advance for your participation.


Nominee Criteria

The nominee must be currently caregiving for someone in the community in OCES’ service area. 

  • Caregiver can be any age caring for an older adult or person with a disability;
  • Caregiver can be a grandparent any age caring for a grandchild (or grandchildren);
  • Caregiver has positively impacted their care receiver’s life or others living in communities within OCES’ service area.
  • A photo (JPEG, GIF, and PNG version) must be provided of Caregiver. (Photos must be emailed to development@ocesma.org stating Caregiver's name in subject line.)


By nominating a caregiver for National Caregivers Month, you are helping us shine a light on their selfless dedication and hard work. We are excited to announce that we will be highlighting our nominated caregivers throughout the month of November through our social media channels and a special Caregivers Edition newsletter.


Prior to November, we will notify the nominated caregivers and share the details of our recognition plans with them. Your nomination is the first step in celebrating these unsung heroes, and we look forward to sharing their stories and honoring their commitment to caregiving.
