Background


This is the application for local businesses to participate in the City’s Catalytic Converter Painting & Etching Program. Successful applicants will receive a $125 reimbursement from the City of Lake Forest for every vehicle serviced pursuant to the program. Please answer each question to the best of your ability. This form should take about 30 minutes or less to complete.
