Fill this form out and submit it. We will invoice you for the Booths and Electricity. You can pay by credit card or mail in a check made payable to Wayne Day, Inc. Applications and payment are due NO LATER THAN August 23, 2024, unless we fill up before that date. Applications received after that date will only be considered if we have room and upon payment of a $100.00 late fee. All exhibit booths are allotted a 12x12 SQUARE unless granted otherwise. We provide the space; you provide the tents, tables, chairs, etc.
