Caregivers As Partners in Care Team (CAP-CT) is a nation-wide initiative that provides health care teams with the skills and confidence to include family caregivers in a patient's care journey. CAP-CT is funded by the Administration for Community Living (ACL) to fulfill Goal 2 of the 2022 National Strategy to support Family Caregivers. CAP-CT is developed and implemented by the University of California, San Francisco, in partnership with leading health care researchers and caregiving support organizations across the country.
