| $10,000 award for successful referrals Terms and Conditions
- Physician must be a Seventh-day Adventist in good standing with their local church
- Physician must not be actively pursued by the Adventist Clinic at the time of referral. Actively pursued is defined as active engagement within a 3-year period in the form of various correspondences.
- Physician must be hired within 3 years of the fully executed agreement between Referrer and Clinic Administration. If Physician is not yet hired, the Referrer may resubmit a referral to renew the 3-year term.
- Physician must not have been referred in the last 3 years by another Referrer.
- Physician referral will be on a first come, first served basis. Only one approved Referrer per Physician at any given time. Should the 3-year term lapse and the Physician is not yet hired, another individual may submit another referrer on a first come, first served basis.
- Referrer must not be an active Board member or a member of the Clinic Administration Team (CEO, Medical Director, HR Director, Director of Finance) at the time of this agreement.
- Upon completion of the form below, the Clinic Administration will contact the referrer for more information
