Media: By accepting the terms and conditions you are agreeing to information provided below and give your full consent as stated.
I authorize COACT and those acting pursuant to it's authority to:
- Record my participation and appearance on videotape, audiotape, film, photography or any other medium.
- Use my name, likeness, voice and biographical material in connection with those recordings.
- Exhibit or distribute such recording in whole or in part without restrictions or limitation for any education or promotional purpose, which COACT and those acting pursuant to it’s authority, deem appropriate.
- Exhibit or distribute any written documentation in whole or in part without restrictions or limitation for any educational or promotional purpose, which the COACT and those acting pursuant to it’s authority, deem appropriate.
Personal Code of Conduct: By accepting the terms and conductions, you agree to uphold the following code of conduct for volunteering.
- I understand and agree that I am a volunteer when participating in all activities of COACT, and I shall receive no payment for my services.
- No contract or agreement of employment is created by any written or oral representations made in connection with the volunteer activity or in connection with any other program of COACT.
- I understand that I am not an employee of the organization, rather a volunteer who serves at the will of COACT. I have no expectation of continuing my participation in the program.
- As a volunteer, I understand that the organization does not provide me with employee benefits, accident insurance, death benefits, compensation for lost time due to injury; nor does the organization carry general liability insurance covering volunteers.
- As a volunteer, I agree to perform to the best of my ability, the tasks as outlined in my job description or the tasks established by my supervisor; report to work on time, when scheduled, and if unable, to call my supervisor; to accept supervision, maintain confidentiality; observe its goals and objectives and give my supervisor adequate notice before termination as a volunteer.
- I will maintain respectful conduct and present a positive image to the community. COACT strives to maintain a workplace that is free from illegal discrimination and harassment.
- While all forms of harassment are prohibited, it is the organization’s policy to emphasize that sexual harassment is specifically prohibited.
- Any volunteer who engages in discriminatory or harassing conduct towards is subject to removal.
- Complaints alleging misconduct on the part of volunteers will be investigated as promptly and as confidentially as possible by COACT.
