Program Volunteer Application

Thank you for your interest in volunteering with Eva’s Heroes Programs. Volunteers are an important part of our team and we appreciate everyone who gives us the gift of their time. We require our program volunteers to be a minimum of 16 years of age. Volunteers under the age of 18 must fill out a Minor Release Form, and Volunteers over the age of 18 must fill out a Background Check Form.
