VOLUNTEER FORM

VOLUNTEER WAIVER AND RELEASE OF LIABILITY in consideration for my participation as a volunteer with Pueblo Action Alliance(Organization Name), hereby agree to the following:

1. VOLUNTARY SERVICE: I understand and acknowledge that my volunteer services are offered freely and without expectation of compensation. I am participating willingly and for the benefit of PAA and its beneficiaries.

2. DESCRIPTION OF SERVICES: I understand that my volunteer activities may include, but are not limited to: assisting with events, office work, direct service to clients, etc.).

3. ASSUMPTION OF RISK: I understand that there are inherent risks and dangers associated with volunteering, including but not limited to: physical exertion, exposure to weather conditions, interaction with the public, use of equipment, etc.). I voluntarily assume all risks associated with my participation in these activities, whether known or unknown.

4. RELEASE OF LIABILITY: I hereby release and forever discharge PAA, its officers, directors, employees, agents, and volunteers (collectively referred to as "Released Parties") from any and all claims, demands, actions, causes of action, liabilities, losses, damages, costs, and expenses (including attorneys' fees) of any kind or nature whatsoever,

5. MEDICAL TREATMENT: I authorize PAA to obtain medical treatment for me in the event of an accident or illness during my volunteer service. I understand that I am responsible for any medical expenses incurred, unless otherwise agreed upon in writing

6. Volunteer agrees by signing this form they are the legal age of (18+) in order to fully participate for INDIGENOUS COMMUNITY DEFENSE.

PHOTO/VIDEO RELEASE: I hereby allow PAA to use images, audio, and video captured during public events, and volunteering efforts, and use without compensation while under services/volunteering with Pueblo Action Alliance.
