National Healthcare Decisions Day

New Advance Care Planning (ACP) initiatives, including from the State of Maryland, are focusing on meaningful community engagement, empowerment and partnerships to increase public awareness about ACP and help normalize ACP conversations and documentation.

Join us for an overview of the new Maryland Department of Health Advance Care Planning Program followed by panel discussion with local providers. Session will include Q&A from audience and conclude with summary of key points and review of community resources.

Webinar is FREE, but registration is REQUIRED.

Together we will:

1)     Understand goals and objectives for new statewide initiative to strengthen Advance Care Planning (ACP) across Maryland

2)    Increase knowledge of barriers to development of Advance Directives

3)     Explore advantages and challenges of current tools available to assist patients, families and medical providers in documenting goals and wishes for care

4)     Identify opportunities and challenges to connect local activities to statewide initiative
