Salem Health Case Management Resource Fair 10/12/26 (set up 11 am)

Vendors/Providers have the opportunity to network with Hospital Case Managers, Social Workers, and Discharge Planning support staff. Hospital staff is able to gather resource information from local providers about theirs services and programs. The funds from registration are donated to support low income patients. Boxed lunches are provided for case management staff.

· LUNCH Sponsor- only 1 space available

You will provide individual box lunches and beverages for the case managers. You will not pay for a table, but only the cost of providing the lunches from a third party commercial licensed kitchen.
