Pre-class Documentation: A DODD Application form must be emailed to info@academyforpd.com by noon 3 business days before the class. Click here for the form. If the DODD application form is not received by 3 business days before class, you (or your staff) will be removed from the class.
ID: Independent Providers must present driver's license to trainer.
Meals: Meals are not included. Due to time restraints we encourage you to pack a lunch.
Registration Deadline: Four (4) business days before the class, or when class is full.
Cost: Free for providers in Lake County.
Cancellation Policy: Cancellations must be received by email at least 48 hours in advance. Thank you for your understanding of our policy. |
