The 2025 LiveWell Douglas County Spring Action Summit will focus on "planting the seeds" to create a community-based health navigator program. Identified as one of the intersecting strategies in the Douglas County Community Health Improvement Plan (CHIP), we have an opportunity to build a program that will support the health and well-being of our neighbors, friends and families.
