Evergreen Health Medical Center Case Management Resource Fair 9/30/25

Vendors/Providers have the opportunity to network with Hospital Case Managers, Social Workers, and Discharge Planning support staff. Hospital staff is able to gather resource information while meeting with providers in person. Space is very very limited.

You will provide individual box lunches and beverages for the full case management team. You will not pay for a table, but only the cost of providing/delivery of the lunches (must be ordered from 3rd party restaurant).
