Pre-class Documentation: A DODD Application form must be emailed to info@academyforpd.com by noon 3 business days before the class. I UNDERSTAND THAT IF THE APPLICATION IS NOT RECEIVED 3 DAYS PRIOR, STUDENT MAY BE REMOVED FROM CLASS. Click here for the form. ID: Independent Providers must present proof of high school graduation/GED and all students must present driver's license to trainer. Meals: Meals are not included. Due to time restraints, we encourage you to pack a lunch. Registration Deadline: Four (4) business days before the class, or when class is full.
Fee: Free for active providers in Miami or other WestCON counties. If the counties determine you do not provide services in one of these counties, you will be removed from the class.
Cancellation Policy: Cancellations must be received by email at least 48 hours in advance, otherwise you will be charged $25.00 for the class. Thank you for your understanding of our policy. |
