The Chestnut Family Health Center Mobile Unit will be providing school physicals to McLean County students entering kindergarten, 6th grade, 9th grade, and new students joining the district! Immunizations will be provided as part of the exam.
A parent/legal guardian must be present and provide identification at the time of appointment.
Please bring to your appointment:
- Insurance information (if applicable)
- Current vaccination/immunization records
Discounted fees may apply based on eligibility.
