Asante Rogue Regional Case Management Resource Fair 10/21/26 @11:30am for set up

Providers have the opportunity to network with Hospital Case Managers, Social Workers, and Discharge Planning support staff. Hospital staff is able to gather resource information while networking with local providers.
*All funds from the registration fee will be donated to the hospital to assist patients in need.

· LUNCH Sponsor- only 1 space available

You will provide individual box lunches and beverages for the case managers. You will not pay for a table, but only the cost of providing the lunches from a third party commercial licensed kitchen.
