Complete the Attached Form:

You can easily complete the referral form attached to our website. This form includes all the necessary information we require to schedule an appointment and provide the appropriate services. Please ensure that all fields are filled out accurately and provide as much detail as possible.

Fax or Email Referrals:

If you already have your own referral form or prefer to use your own template, we gladly accept those as well. Simply fax the completed referral form to 888-813-6815 or email it to hr@gbehavioral.com. Our dedicated team will review the referral promptly and take the necessary steps to schedule the appointment.

What to Expect After Sending a Referral

Once we receive your referral, our team will carefully review the information provided. We understand the urgency and importance of timely communication, so we strive to update the referring provider as soon as possible.

Appointment Confirmation: We will contact the referred individual or their guardian to schedule an appointment at their earliest convenience. Our team will work closely with the client to find a suitable time that accommodates their schedule.

Declined Services Notification: In the event that the referred individual declines services, we will promptly inform the referring provider. We understand that collaboration and communication are crucial, and we aim to keep all parties involved informed throughout the referral process.
