You are eligible for your provider’s care management program, ChartSpan! This program includes a care manager to help you achieve your health goals and a medical device, shipped to your home, that sends important clinical data to your care manager and provider. You can use your device to track your daily health readings, which will inform your care manager’s monthly check-ins. They will also see your readings between calls so they can follow up on changes that need attention.

While the device will not have a cost, there may be a cost for the monitoring services. This program will be referred to as “remote monitoring” on insurance bills or statements.
