9:00 AM to 11:30 AM

What is the Class?

Chronic Disease Self-Management Program (CDSMP) is for adults with a variety of chronic health conditions. The program may be particularly beneficial for people who have more than one health condition, whose health problems have begun to interfere with their valued life activities, or who have had difficulty following their health recommendations. Caregivers are welcome at the workshops and are encouraged to participate as well. 


Topics covered include: nutrition, appropriate exercise, communicating effectively with family, friends and health care providers, appropriate use of medications, pain and fatigue management, relaxation skills, dealing with depression, decision making, action planning and problem solving.

What Can I Expect During Class?

CDSMP, a community-based class, is an interactive and participatory program for adults who are living with chronic diseases.
