Q & A- Below are the questions and answers submitted by March 29th.


1.It appears that there are two different programs at our agency that are planning to apply for this funding and I know that there was a disclaimer in the presentation that we could do that, but only one may be awarded. I am working with the two individuals to see if they are interested in applying together or not but it may help guide that conversation if I understood this a little better. I am under the impression that both of these programs have someone in mind to hire as a CHW, but neither of them is certified. If we were to be awarded funding for one (or both) of these projects - I see that there is additional funding available for Micro-Credential training, but is there anything to help cover the initial certification? I looked online and see that it is about $1500 plus the cost of the book for that initial certification and when we meet to discuss further I want to be able to discuss how that would be paid for with other funds if necessary.

a.     Whether we can award for both programs would depend on the total amount requested by all applications. I would recommend submitting for both programs and if awarded for only one we could discuss details.

b.     We will cover the initial training cost of new CHWs for those that are not already certified. This would NOT be taken out of your funds. We utilize Everyday Life Consulting for our certification training- https://everydaylifechw.com/shop/chw-training/nchw/ They offer at your own pace training, zoom live training, and hybrid. If we have 10 or more new people to be trained, we may arrange for an in person option.

2.      I do see some interest in the chronic disease piece of it, but we would potentially be limited to working with school-age children due to the nature of our organization. Would that be a problem?

a.     You have the option of working with the parents and caregivers of the children that attend your school. Working with people already diagnosed to manage their chronic disease is a core activity of the original funder from which these funds are available. If you would like to provide the Chronic Disease portion, you need to have some services for adults. Another target population could be school staff who have chronic conditions. You could supplement with some classroom chronic disease prevention education or two generation programming such as cooking classes. 

b.     If you did not provide the chronic disease portion of the RFP then you would only complete deliverables 3 and 4:

                                                                     i.      Participation in CHW Navigation Services (5 hrs/week) $5,000

                                                                   ii.      Progress to initiate or enhance billing for CHW services $5,000

You would be applying for Navigation only CHW.

3.     Does an organization have to train or hire a new CHW for this program? Or is a current CHW able to participate?

a. You do not have to hire and train a new CHW for this program. You could utilize a current CHW if you meet the deliverables. The chronic disease management programs you would be implementing would need to be new. We are not allowed to supplant existing funding/programs.

4.     Is there a specific Chronic Disease training program that would be preferred to qualify for the deliverable? What commitments would the training involve?

a.     We are aiming to have all CHWs trained in the Evidence Based Chronic Disease Self-Management program. Details on training can be found at https://selfmanagementresource.com/training/ We will cover all costs for training and licensing during the project. These costs will NOT come out of your awarded funds. https://selfmanagementresource.com/licensing/

b.     Please note we have budgeted for a few extra professionals to be trained which could include other people in the care team. We have also budgeted to have 2 people become master trainers to be able to train additional leaders moving forward. https://selfmanagementresource.com/training/program-training/virtual-master-trainings/

c. In addition to a Chronic Disease Self-management program, we are also open to other suggestions or ideas for training of CHW and Care Team. 

5.     What are the approved Chronic Disease strategies that we could implement?

a. We would like everyone to utilize their training by providing at least one Chronic Disease Self-management program. There are many options available- https://selfmanagementresource.com/programs/

                                             i.    Face-to-face small groups

                                           ii.    Virtual online small groups 

                                         iii.    Mailed tool kits with weekly small group

                                         iv.    Internet asynchronous groups

b. In addition to a Chronic Disease Self-management program, we are also open to other suggestions or ideas for evidence based programming that you would like to try. The funding has encourage diabetes and hypertension management programs.
