IMPORTANT REMINDERS


  • This is the one time per year employees can make changes to their benefit elections.


  • The elections you make now are generally irrevocable unless you experience a qualifying life event.


  • If you waive coverage now, you cannot enroll until the next open enrollment.

Important Information

If You Experience a

Mid-Year Qualifying Event







You may be entitled to change your election if you, your spouse, or dependents experience one

of the Qualifying Events below.


  • Divorce or legal separation
  • Open Enrollment for Spouse's employer
  • Change in number of dependents
  • Change in employment status that causes loss of eligibility
  • Dependent satisfies or ceases to satisfy eligibility requirements
  • Change in coverage under another employer plan
  • Loss of coverage from government or educational institution
  • COBRA qualifying event (termination/reduction of hours, employee death, divorce/legal separation, ceasing to be dependent)
  •  Other changes resulting from a judgment, decree, or order; Medicare or Medicaid entitlement


Elections MUST be made within 30 days of your qualifying event

Proof of Qualifying Event may be requested to process your enrollment.

All current Elections for Dental, Vision, and Voluntary Life will roll over to the new plan year. New enrollment forms only need to be completed if you are newly enrolling, changing your current coverage or waving coverage.


Enrollment forms need to be received by the enrollment deadline.

All full-time employees must either elect or waive coverage no later than two weeks prior to your effective date.
