Four new indicators were added to the Service Delivery and Documentation Tracking category across the Provider Level, Building Level, Scattered Site Level, and Case Manager Level Summary tabs. The additional indicators include:
- Demographic Profile Updated Within the Past Year – Displayed as a percentage. Represents the proportion of active cases with a Demographic Profile update within the past year.
- Household Composition Profile Updated Within the Past Year - Displayed as a percentage. Represents the proportion of active cases with a Household Composition Profile update within the past year.
- Medical Profile Updated Within the Past Year - Displayed as a percentage. Represents the proportion of active cases with a Medical Profile update within the past year.
- Legal Profile Updated Within the Past Year - Displayed as a percentage. Represents the proportion of active cases with a Legal Profile update within the past year.

Two new indicators were added to the Service Delivery and Documentation Tracking category in the Participant Level Summary tab. The additional indicators added include:
- Most Recent Medical Profile Update – Indicates the most recent occurrence of the “Save” button being clicked on the Medical Profile page in an ICMS participant’s CHAMP profile (includes self-reported physical and behavioral health information, mobility and accessibility needs, medical and mental health conditions, and ADLs/IADLs). Clicking the “Save” button represents the case manager’s attestation that the participant’s Medical Profile information has been reviewed, updated as needed, and confirmed.
- Most Recent Legal Profile Update – Indicates the most recent occurrence of the “Save” button being clicked on the Legal Profile page in an ICMS participant’s CHAMP profile (includes self-reported conviction history and other legal challenges). Clicking the “Save” button represents the case manager’s attestation that the participant’s Legal Profile information has been reviewed, updated as needed, and confirmed.
It is expected that each of these profile sections is reviewed and updated or confirmed at least once annually, and whenever significant changes occur that should be reflected in the participant’s CHAMP record. If the most recent save action occurred more than one year ago, or reflects only the initial creation of a skeleton participant profile to initiate enrollment, the cell will be highlighted in salmon to indicate that an update review is needed. A new indicator was added to the Current Resources and Statuses category in the Participant Level Summary tab. The additional indicator added includes:
- Universal Housing Application (UHA) Status – Indicates the most recent Universal Housing Application (UHA) status on record for the participant, when applicable. For participants who are not yet permanently housed, or who are permanently housed and have a subsidy exchange or relocation request in process, this indicator provides a brief summary of the most recent UHA step completed. This information can be used to determine whether further review is needed in the UHA Log and to support targeted housing navigation efforts.
The UHA Log tab was added to the ICMS Snapshot. This tab provides a list of all Universal Housing Applications (UHAs) on record for active ICMS participants. Each row represents a unique UHA. For each UHA, the tab includes housing resource details, as well as statuses, dates, and notes for each step in the UHA process. The UHA Point of Contact (POC) on record is also included. This information can be used to support targeted housing navigation efforts and timely follow-up on outstanding UHA steps. |

- This tab provides a list of all ICMS trainings completed by each active case manager assigned under a provider’s ICMS contract. It displays the name of each training completed and the associated completion date, helping providers monitor training participation and ensure staff remain up to date on ICMS program guidance.

- TLS? - Indicates whether the ICMS participant is currently associated with a Time-Limited Subsidy (TLS) enrollment based on available data. Possible values include:
- Blank – No active TLS enrollment is detected
- Yes – Not Matched to PSH – Participant is enrolled in TLS but not matched to a permanent supportive housing (PSH) opportunity
- Yes – Matched to PSH – Participant is enrolled in TLS and has been matched to a PSH opportunity
- Yes – Exchanged to PSH – Participant has transitioned from TLS to a PSH opportunity

- This tab provides a summary view of all scattered site ICMS slots active under a provider’s contract. It displays aggregated metrics related to program enrollment and billing, staffing, service delivery and documentation, participant resources and statuses, and care team coverage. The information in this tab can be used to support program oversight and care coordination for scattered site ICMS operations.

A new indicator was added to the "Care Team" category in the Participant Level Summary tab. The additional indicator added includes:
- Caregiver - Indicates whether the ICMS participant is actively enrolled in a caregiving program. This is intended to eventually reference both IHSS and IHCG enrollment records, but currently only references IHCG enrollment records due to information access limitations. If the cell is blank, it indicates that either the participant is not actively enrolled in in caregiving, or enrollment information is not being detected in available records, or there is not adequate sharing consent in CHAMP to share this information out. If there is adequate sharing consent in CHAMP and the participant is actively enrolled in a caregiving program according to available records, the cell will display the caregiving program information.

| A new indicator was added to the "Alerts" category in the Participant Level Summary tab. The additional indicator added includes:
- Health Care Action Step Needed? - Indicates whether the ICMS participant is in need of a high priority health care action step from ICMS based on currently available information. If the cell is blank, it indicates that either the participant is not necessarily in immediate need of a high priority health care action step from ICMS according to available records, or there is not adequate sharing consent in CHAMP to share this information out. If there is adequate sharing consent in CHAMP and the participant is in immediate need of a high priority health care action step from ICMS, the specific action step needed will be displayed. There is currently only one possible health care action step being displayed for this data point: "TOC Visit Needed." The cell will display TOC Visit Needed if there is a record of an emergency department or inpatient visit within the past two weeks, but no "ICMS TOC Visit" Service recorded in CHAMP. The data cell also contains a clickable link to the Transitions of Care landing page within the ICMS Information Center to provide the most currently available guidance on this area of ICMS.

Three new indicators were added to the "Identity and Location Information" category in the Participant Level Summary tab. The three additional indicators added include:
- Household Size - The current number of unique household members listed in the Household Composition section of an ICMS participant's CHAMP profile.
- Dependents - The current number of unique household members listed in the Household Composition section of an ICMS participant's CHAMP profile who have a "Dependent" relationship to the head of the household. Dependent household members may or may not be minors (under age 18).
- Minors - The current number of unique household members listed in the Household Composition section of an ICMS participant's CHAMP profile who have a birth date less than 18 years ago. Minors in the household may or may not be dependents of the head of the household.

- Geographic Preference(s) for Housing - This indicator lists the most recent update of the geographic preference(s) selected in the Housing Preferences section of an ICMS participant's CHAMP profile. This information is used to help inform housing matches and housing navigation support efforts for participant's who are not yet permanently housed. If the most recent update to the list of geographic preference(s) was more than one year ago, it is considered expired and will display as blank in the ICMS Snapshot.
- Mobility/Accessibility Needs - This indicator lists the most recent update of the accessibility/mobility needs selected in the Medical Profile section of an ICMS participant's CHAMP profile. This information is used to help inform housing match eligibility and housing navigation support efforts for participants who are not yet permanently housed, or to support those seeking reasonable accommodation. If the most recent update of a participant's mobility/accessibility needs in their CHAMP profile was more than one year ago, it is considered expired and will display as blank in the ICMS Snapshot. If universal information sharing consent is missing (including expired), declined, or revoked, this data point will display as "Consent Needed."
- Disabilities - This indicator lists the most recent update of the disabilities selected in the Physical Health Information subcomponent of the Medical Profile section of an ICMS participant's CHAMP profile. This information is used to help inform housing match eligibility and housing navigation support efforts for participants who are not yet permanently housed, or to support those seeking reasonable accommodation. If the most recent update of a participant's disabilities in their CHAMP profile was more than one year ago, it is considered expired and will display as blank in the ICMS Snapshot. If universal information sharing consent is missing (including expired), declined, or revoked, this data point will display as "Consent Needed."
- Living with HIV/AIDS? - This indicator displays a "Yes" if HIV/AIDS is listed as a health condition in the Health Conditions section of an ICMS participant's CHAMP profile. This information is used to help inform housing match eligibility and housing navigation support efforts for participants who are not yet permanently housed. If universal information sharing consent is missing (including expired), declined, or revoked, this data point will display as "Consent Needed."
- Prior Criminal Justice System Involvement? - This indicator displays whether ("Yes" or "No") history of a "legal issue" has been listed in the Legal Profile section of an ICMS participant's CHAMP profile. This information is used to help inform housing match eligibility and housing navigation support efforts for participants who are not yet permanently housed. If a participant's Legal Profile section of their CHAMP profile was never updated since profile creation, it will display as blank in the ICMS Snapshot. If universal information sharing consent is missing (including expired), declined, or revoked, this data point will display as "Consent Needed."

| Six demographic indicators were added to the "Identity and Location Information" category in the Participant Level Summary tab. The six demographic indicators added include:
- Date of Birth
- Citizenship Status
- U.S. Veteran?
- Primary Language
- Gender
- Race and Ethnicity
If any of these indicators are either missing or logged as "Data Not Collected" in CHAMP, the cell will highlight salmon, indicating this demographic information is outstanding in CHAMP. |

"Most Recent Health Care Accompaniment" was added as a new indicator in the "Service Delivery and Documentation Tracking" category of the Participant Level Summary tab. This represents the most recent occurrence of an accompaniment to a health care appointment with the participant by the assigned ICMS Provider. This is determined by referencing service dates for all Services recorded as "Accompaniment - Health Care." This check can only go back up to one year. If the most recent health care accompaniment was recorded over one year ago, or if there has never been a health care accompaniment Service recorded by the assigned ICMS Provider, this field will display as blank. If there has been a health care accompaniment Service recorded by the assigned ICMS Provider within the past year, the date of the accompaniment will be displayed. |

The "Open House Visit Recommended for this Unhoused TBV Holder?" column in the Participant Level Summary of the ICMS Snapshot has been retitled to "Housing Navigation Action Step Needed?"
An indicator link that states "Attend Open House" will still appear in this column, as before, for participants who are identified in CHAMP as searching for housing and eligible for the linked housing opportunity. An additional indicator link that states "Submit FHSP Application" has been added. This link will appear for participants who are identified in CHAMP as unhoused and matched to an FHSP subsidy, but there is no record of an FHSP application submitted. ICMS Providers are expected to submit FHSP subsidy applications to Brilliant Corners within 14 days after match for project-based vouchers (PBVs), or within 21 days after match for tenant-based vouchers (TBVs). Failure to submit an FHSP application within these timeframes may result in the participant being exited from ICMS by HFH staff to ensure timely voucher utilization. |

The Universal Sharing Consent Status data point still indicates the most current information sharing consent status on record in CHAMP, including:

- Limited SUD/MH Data Sharing Provided - No MH or SUD Info
- Revoked
- Declined
However, we updated our flagging logic to better align with ICMS program policies, as well as to incorporate results from our manual consent document reviews. There are additional Universal Sharing Consent Statuses that may be displayed in the ICMS Snapshot, and what each indicates:
- Expiring: If the sharing consent status is 'Full' or 'Limited' but will expire within the next 60 days.
- Error(s) in Uploaded Document: After HFH's manual review of the uploaded document, possible errors identified in the document include one or more of the following: boxes not checked to match consent status selected, CID not written on document, DOB missing, initials missing, old consent form, signature missing, signature date missing, poor image quality of scanned document.
- Status Not Matching Document: While rare, some consent statuses are marked as 'Limited', but the scanned document indicates a 'Full' consent status should be assigned.
- Blank: Indicates there is either no consent status on record, or a previously 'Full' or 'Limited' consent has expired, or a Revoked or Declined consent status is more than 6 months old. A Declined or Revoked status will only remain valid for 6 months, as HFH expects ICMS providers to revisit this discussion with a participant every 6 months when there is a Declined or Revoked status.

If there is adequate sharing consent in CHAMP and the participant is actively enrolled in mental health services under DMH, the cell will display the DMH program name, and the name of the assigned provider. |

The Emergency Room or Inpatient Visit Within Past 2 Weeks? data point indicates a "Yes" if at the time the ICMS Snapshot is generated, the ICMS participant entered or discharged from an emergency room and/or inpatient hospital facility in LA County within the past two weeks.
The Emergency Room Visits Within Past 6 Months data point provides a tally of the number of encounters the ICMS participant had with an emergency room facility in LA County within the past 6 months.
The Inpatient Visits Within Past 6 Months data point provides a tally of the number of encounters the ICMS participant had with an inpatient hospital facility in LA County within the past 6 months.
The Hospitalization Log is a list all emergency room and inpatient hospital encounters for ICMS participants who have had one of these types of encounters within the past two weeks. The data points included in this log are encounter type, encounter start date, encounter discharge date, and encounter location. A blank discharge date for an inpatient hospital encounter may indicate that the participant is still receiving care at that facility.
**Disclaimer: All emergency room and inpatient hospital encounter data shared in this file may be limited or incomplete. Emergency room and inpatient hospital data, when available, are only shared in the ICMS Snapshot when full information sharing consent and a signed notification of DHS’ Privacy Practices are present in the participant’s CHAMP profile.

The snapshot temporarily includes a new column at the end of Client Level Summary titled Open House Visit Recommended for this Unhoused TBV Holder? A link that states "Attend Open House" will appear in this column for participants who are identified as eligible, according to CHAMP data.
By following the "Attend Open House" link, you can access key details about a building with almost 200 available housing units that needs to be filled by the end of February. (Please note, upon clicking this link for the first time, a warning message may appear; however, it is safe to ignore this warning.)
This update ensures that participants who qualify for a space in this building, if they have an housing voucher in hand, can be promptly identified and directed to this opportunity.

The Brilliant Corners Housing Coordinator indicator highlights which active ICMS participants in your CHAMP roster also have an active enrollment in HFH's Property-Related Tenant Services (PRTS). Brilliant Corners (BC) housing coordinators provide PRTS.
When this data cell is blank, it indicates the ICMS participant is not enrolled in PRTS with HFH.
When the data cell for this indicator is populated, it indicates the ICMS participant is enrolled in HFH's PRTS:
- If an ICMS participant is enrolled in PRTS and there is a BC housing coordinator assigned, the data cell lists the BC housing coordinator's name and email address.
- If an ICMS participant is enrolled in PRTS but there is not a BC housing coordinator assigned yet, "Awaiting Assignment" is displayed in the data cell.

The Staying in DHS Interim Housing? data point has been updated. Previously, it would indicate a "Yes" if at the time the ICMS Snapshot was generated, the ICMS participant was actively checked in to a DHS interim housing project. And if that data point was blank, it was an indicator the ICMS participant was not actively checked in to a DHS interim housing project.
With the new update, a blank still cell indicates the participant is not actively checked in to a DHS interim housing project. However, now when a participant is actively checked in to a DHS interim housing project, instead of a "Yes," the ICMS Snapshot displays the name of the interim housing provider and the facility. |

Each row represents a unique ICMS enrollment that was ended as part of a program exit, and includes information on which participant was excited, which particular slot was vacated, the check-in and check-out date from the slot, and available information on the reason for the ICMS exit. |

The Notification of Privacy Practices on Record? indicator highlights which active ICMS enrollments in CHAMP have record of an ICMS participant receiving an overview of DHS Privacy Practices. A "Yes" indicates there is a Notification of Privacy Practices on record and that another one is not needed. "Declined" indicates that the participant declined to sign the Acknowledgement of the Notice of Privacy Practices form the last time it was reviewed with them. It is advised for ICMS practitioners to periodically check back in with participants who previously declined to acknowledge DHS Privacy Practices, in case the participant changes their mind. When this data point is blank, it indicates there is no record in CHAMP that DHS Privacy Practices have been reviewed with the ICMS participant, and it is advised that the ICMS practitioner reviews this information with the ICMS participant ASAP.. |

The Services Log tab added in the ICMS Snapshot is a point in time summary that lists all Services recorded in Case Notes within the current month of service for all currently active ICMS participants. Each row represents a unique Service that was recorded and includes information on where it was provided, when it was provided, when the record was created in CHAMP, and which CHAMP user created the record. The Action Step Log tab added in the ICMS Snapshot is a point in time summary that contains all historical Care Plan content recorded in CHAMP (including all open and closed SMART Goals) for currently active ICMS participants. The information in the Action Step Log can be used to track case progress and service quality across your participant roster, and to improve care coordination. Each row represents a unique Action Step within the ICMS participant's Care Plan. For each Action Step, this Log indicates the SMART Goal it was created under, all relevant information about that connected SMART Goal, the current status of the Action Step, which CHAMP user created the Action Step, and which CHAMP user most recently updated the Action Step. Action Step Log data can be filtered in many ways, but two common strategies include filtering by Case Manager to isolate Care Plan data for a particular case manager’s caseload, and by CHAMP ID to isolate these data for a particular participant. |

The Checked in 6+ Months and Not Yet Permanently Housed? indicator highlights cases with an ICMS project check-in date from 6 or more months ago, but no data in the PH Update indicating the participant has become permanently housed yet. When this item is noted as “Yes” in the Documentation Snapshot, DHS requests the case manager reviews the participant's case to ensure all appropriate housing navigation and move-in data are added in the PH Update. It’s important to closely monitor whether participants have been unhoused for too long after project check-in. Subsidy opportunities can expire and DHS needs to be made aware of whether a match to a new housing opportunity is needed. |

The CalAIM CS Housing Navigation (HN) or Tenancy Sustaining Services (TSS) Verbal Opt-In Needed? indicator highlights ICMS participants for which a CalAIM HN/TSS Opt-In is missing in the CHAMP profile. When this item is flagged in the Documentation Snapshot as being needed, it is indicating that DHS requests the case manager to collect the verbal CalAIM CS Opt-In from the participant and record it in CHAMP ASAP. Collecting the verbal Opt-In enables DHS to utilize a participant's Medi-Cal, when available, to help pay the ICMS provider for the support they are providing the participant within the ICMS program. The Family Billing Rate But Dependent Household Member(s) Not Listed in Household Composition? indicator highlights ICMS enrollments which are currently assigned a family billing rate, but the Household Composition section of the participant's CHAMP profile either only has one household member listed and/or none of the household members is labelled as a "Dependent." For all ICMS enrollments with a family billing rate, it is required that the Household Composition section of the ICMS participant's CHAMP profile lists all household members, and that the dependent(s) are labelled, accordingly. ICMS enrollments with a family billing rate that are missing the required household composition information for 30+ days will be redesignated to a single adult billing rate.
The # "Eligible" Services Recorded for Last Month data point has been retired from the ICMS Documentation Snapshot report template. Staying in DHS Interim Housing? is a new data point to help point out which participants in your roster are currently staying in a DHS Interim Housing facility. This information should be used to guide updates in the Care Plan that help promote further collaboration between you and the Interim Housing facility, and to enhance permanent housing navigation coordination. |

The Medi-Cal Status indicator has been enhanced. This data point is still based on Medi-Cal data we receive once per month. We can only share information on a participant's Medi-Cal coverage status if there is a limited or full sharing Universal Consent on file. If there is not adequate Universal Consent on file, the status will be displayed as "Consent Needed". When there is adequate Universal Consent on file, these are the possible statuses that can be displayed:
- An "Active" coverage status indicates the participant currently has active Medi-Cal coverage that is not expiring soon according to our best understanding at this time.
- An "Expiring Soon" status indicates the participant's Medi-Cal coverage is currently Active, but it is at risk of expiration within the next 60 days, and so updating the Care Plan to focus on immediate Medi-Cal renewal is recommended.
- An "Investigate" status indicates we do not yet have clear enough information to indicate the participant's Medi-Cal coverage status. It is recommended to update the Care Plan to investigate the participant's Medi-Cal coverage status and take appropriate action to ensure active coverage.
- An "Inactive" coverage status indicates the participant does not currently have active Medi-Cal coverage. It is recommended to update the Care Plan to investigate the participant's health insurance coverage situation and help them to reestablish Medi-Cal ASAP if they do not have an alternative active insurance plan.

Main Income Source (Amount) displays the participant's highest paying income source. If this data point is blank in the Snapshot, it indicates either no income data has ever been logged in the participant's financial profile, or it has been over one year since the last income confirmation in CHAMP. Please ensure this information is correct. |
