Diabetes Self-Management Education and Support (DSMES)

Diabetes Self-Management Education and Support, or DSMES, teaches participants how to eat healthy, be active, monitor blood sugar levels, take medication, problem solve, reduce risk for other health conditions, and cope with their disease. It is evidence-based and improves clinical outcomes, health status and quality of life. We encourage you to review the resources given below for you and your patients. Updates From Your Practice Transformation Coaches

Greetings from the HCI / Beacon Practice Transformation Team! Our hope is that the New Year has begun on a healthy note!  

Sharing a few quick highlights: 

Practice Transformation Incentive Payment Program for PCMPs~~

All PCMP’s will be eligible to receive additional funding for contributing to the successful achievement of RAE initiatives within our region. All PCMP’s will be eligible to participate in the Practice Transformation incentive opportunity. Payments will be dependent on availability of funds. Payment is limited to one location per practice.

Practice Transformation is the implementation of quality improvement tools and evidenced based best practices for primary care. The Practice Assessment is used to understand where a practice is in the practice transformation journey. Your PT Coach will meet with you virtually and the Practice Assessment takes about an hour to complete. After completing the Assessment your Coach can work with you to develop Quality Improvement (QI) Plan for 2021 and work on a PDSA.

The Health Colorado Practice Transformation Coaches Tina Gage and Nate Koller can offer practice transformation support to you to improve performance. Please contact Nate or Tina and they will assist in scheduling a time to with you.

Looking to implement team-based care?

The Colorado Health Foundation is offering funding opportunity to support safety net clinics in receiving technical assistance (TA) to improve their clinical and business capacity so they can offer high-quality, team-based comprehensive primary care. The Foundation will fund clinics up to $125,000 over the two-year period to support clinic staff participation in the funding opportunity and any education and travel-related expenses associated with implementing team-based care.

Primary Care Medical Provider Participation in a Youth Suicide Research Study

Partners For Children's Mental Health is looking for primary care medical providers who see patients ages 10-24 to participate in a 12 month research study. The purpose is to understand the impact and providers' perspectives on implementing a youth suicide care pathway in the primary care setting. Click here for the flyer with more information.

New ACH Enrollment Form for Health Colorado

Beacon Health Options is now a subsidiary of Anthem and has a new ACH Enrollment for Health Colorado, Inc. providers. If you are interested in receiving your future payments electronically, you will be required to set up a new account through ACH Electronic Funds Transfer (EFT) using this form New Enrollment Form for Health Colorado. While front-line workers have had top priority access to COVID-19 vaccines, some may be hesitant to receive shots. These doubts come as a surprise to many who assumed health professionals would eagerly accept one of the COVID-19 vaccines. Feds Announce Open Enrollment

President Biden issued an Executive Order that re-opens enrollment of the Affordable Care Act marketplaces. While the order pertains to states that use the federal exchange (, Colorado’s exchange (Connect for Health Colorado) will mirror the order by also re-opening enrollment. In fact, Coloradans without health insurance will be able to enroll in coverage through between Feb. 8 - May 15, 2021.
