Care and preparation are the keys to success, and our CMH family medicine clinics are here to ensure your children are prepared for a healthy sports season through in-clinic sports physicals and wellness exams.

If your student participates in middle and high school athletics, cheerleading/spirit squad or marching band, they must complete a pre-participation physical evaluation form before starting a camp or practice. Missouri State High School Athletics Association (MSHSAA) requires the forms before the first practice. Physicals are administered at CMH clinics by board-certified physicians, nurse practitioners and physician assistants.

CMH Clinics recommends that your student-athlete complete an annual wellness examination and include the sports physical in the appointment. Most insurance plans cover wellness examinations once per calendar year. All sports physical paperwork can be completed during the wellness appointment. Also, this appointment can be scheduled at your clinic when it is convenient for you and your family, and is not limited to specific dates/times.

If you only need to complete a sports physical (or your student has already completed an annual wellness exam this year), your student-athlete can receive a free sports physical at your family's clinic during sports physical days. Check below for specific dates, times, locations and information for your clinic's designated day(s). (Note: Sports physicals are free on specified dates and times only. Unless otherwise noted, the fee for a sports physical exam is $44 on all other days and times and is due at the time of service.)
